Who can join?
The
Phoenix Club aims to attract individuals from the following
groups.
- Individuals who already exhibit symptoms of Coronary Heart Disease and are
currently undergoing treatment OR have previously undergone
treatment advised by their general practitioner or a
specialist.
- Individuals who have undergone any type of intervention
(surgical or otherwise) as a result of Coronary Heart Disease.
- If capacity allows we can accept inactive (sedentary) individuals with a potential risk
factor profile for Coronary Heart Disease. This might
include the following: overweight, elevated blood pressure,
stress, smoker, family history of heart disease. However,
priority
will be given to patients with existing Coronary Heart Disease.
How do I Join?
- You can be referred from a hospital based Phase III Cardiac Rehabilitation programme.
- You can get a referral from your GP - the referral forms are available to
all GP's online.
- You can self-refer by filling in the on-line form or by phoning us for
more information.
- You can be referred through your cardiac nurse specialist.

Registration with the Phoenix Club
The next pre-exercise assessment takes place on the 15th August. You can only
register on this night. Once you have been referred
to us, we will contact you to book you in to our next available assessment
session.

What will it cost?
The Phoenix Club is a non profit making Club. We charge a nominal fee to
offset some of the costs. All of our staff are volunteers but we do incur some
set costs (hall hire, equipment, stationary etc). We have set the initial fee at
£5.00 per session payable at each session.
